Dear Patients:

I hope this letter finds you in good health. After much consideration, I have made the difficult decision to retire from my medical practice. It has been a great privilege to serve as your physician, and I am truly grateful for the trust you have placed in me over the years. My last day of practice will be 5/31/24. I want to assure you that your medical records will be securely maintained and can be transferred to a new healthcare provider upon your request. Your medical records are confidential and a copy can be transferred to another provider or released to you or to another person you designate only with your permission. To request records please email the request to caroline_neuro@satx.rr.com. I recommend that you establish a relationship with a new provider as soon as possible to ensure continuity of care.  I want to thank you for allowing me to be part of your healthcare journey. It has been a pleasure getting to know you, and I will always cherish the memories and trust you have given me. Wishing you the very best of health and happiness in the future.
